IAAW: Incident Management Awareness | ½-day In-Class

An additional non-Investigator module for leaders, managers, legal advisors and other key figures who are not part of the investigation team, but who do have a role to play in the process of learning from incidents.

IAII: Investigation & Interview Techniques | 2-days In-Class

Ideal for investigators who want to receive investigator soft skills, advanced investigation & interview techniques and knowledge on how to design and execute a complete incident investigation process.

BFA1: Barrier Failure Analysis (Basic) | 1-day In-Class

The training program offers the knowledge and skills required to compose a full BFA analysis. Participants will also learn how to draw conclusions and compose recommendations.

BFA2: Barrier Failure Analysis (Advanced) | 2-days In-Class

This 2-day BFA course is similar to the 1-day course (BFA1). Additional focus is on the facilitation of the BFA process and presentation of the analysis.

RCA1: Root Cause Analysis incl. 5-Why (Basic) | 1-day In-Class

The training program offers the knowledge and skills required to compose a Root Cause Analysis (RCA) incl. 5-Why. Participants will also learn how to draw conclusions and compose recommendations.

RCA2: Root Cause Analysis (Advanced) | 2-days In-Class

This 2-day RCA course is similar to the 1-day course (RCA1). Additional focus is on the facilitation of the RCA process and presentation of the analysis.

IASW: IncidentXP Software (BFA / RCA / TOPSET) | 1-day In-Class

The program focuses entirely on the application of one of the modules of the IncidentXP software suite (BFA / RCA / TOPSET).

IARF: Refresher Workshop | 1-day In-Class

After a brief refresher of the theory, participants will mainly practice using own cases.

IACS: Post-Training Coaching & Support

Our post-training support aims for coaching BowTie practitioners and facilitators during their first experiences in the field.

We also deliver our courses online in a highly interactive Virtual Classroom setting!

  • Incident Management Awareness

    IAAW

    ½-day course

    • This workshop is an additional non-Investigator module for managers, legal advisors and other key figures who are not part of the investigation team, but who do have a role to play during incident investigations and follow-up/implementation of recommendations.
       

    • It is important that leaders at any level in the organisation are familiar with the principles of 'Learning From Incidents' (LFI).
       

    • Delegates will become aware of their role and influence in the overall Incident Management process.

  • Investigation & Interview Techniques

    2-day course

    • This course focuses on the following topics: what are the objectives of an incident investigation (and what not!); Insight into the incident investigation process; Organizing an investigation; Collecting data / evidence; Interview skills & techniques; Drafting a timeline; Communication and reporting both the progress and the findings of the incident investigation; Drawing conclusions; Recommendations; Reflective learning.
       

    • After this course, participants will be able to engage as an investigator/ facilitator in the process of incident investigation.
       

    • This course is ideal for those investigators who want to receive advanced investigation skills and knowledge about the investigation process.

    • Barrier Failure Analysis (BFA) is a pragmatic barrier-based incident analysis method. The BFA structure offers a sequence of events, barriers and causation paths.
       

    • The training program offers the knowledge and skills required to compose a full BFA analysis. Participants will also learn how to draw conclusions and compose recommendations.

  • BFA - Barrier Failure Analysis (Advanced)

    2-day course

    • This 2-day BFA course is similar to the 1-day course (BFA1).
       

    • Day 2 is all about practising using own case studies.
       

    • This 2nd day is highly appreciated by participants. The extra day of practising offers increased trust and advanced skills in the application of BFA.
       

    • Additional focus on the facilitation of the BFA process and presentation of the analysis. 

  • RCA - Root Cause Analysis
    incl. 5-Why (Basic)

    RCA1

    1-day course

    • RCA is a very accessible and straightforward incident analysis technique. It starts with an incident and drills down (by keep asking 'why?') into the chain of events that led to that incident until the root causes are identified.
       

    • The training program offers the knowledge and skills required to compose a Root Cause Analysis (RCA).
       

    • Participants will also learn how to draw conclusions and compose recommendations.

  • RCA - Root Cause Analysis (Advanced)

    1-day course

    • The program focuses entirely on the application of one of the modules (BFA / RCA / TOPSET) of the IncidentXP software suite.
       

    • Note: All regular BFA and RCA courses are mainly focused on the methodology and not so much on the use of the software. During these courses, the trainer does show a quick-tour of the software and demonstrates how an analysis diagram can be composed using the software. However, many other functionalities are not covered. That is why this 1-day software training is an ideal addition to the regular BFA and RCA training courses.

  • Refresher

    1-day course

    • Accompanied by an experienced lead Investigator / accredited trainer, we offer trained BFA / RCA practitioners the opportunity to refresh their knowledge of the method.
       

    • During the workshop, there is ample opportunity to share knowledge and experiences with colleague-investigators. After a brief refresher, we mainly practice using your own cases.
       

    • This course is a refresher. That means that candidates must have previously participated in a BFA / RCA course.

  • Post-Training
    Coaching & Support

    • Our post-training support aims for coaching members of your internal investigation pool.
       

    • Coaching can be done on-site, or remotely via phone, (video) conference call or mail. Also, we facilitate periodic workshops in which the group will be brought together to work on case studies, challenges and specific themes.
