Although organisations have an incident investigation structure in place, it shows not always easy to have that optimally utilised in practice. As a result, the quality of the investigation findings (underlying causes and conclusions) and the quality of the investigation report does not always meet the level of expectation. And from there, ‘learning from incidents’ is bound for a challenging journey.
One is easily tempted to link the dissatisfying quality of the investigation outcome back to the efforts of the investigation team involved.
